How to Register

1. Click on the appropriate link below
2. Create your Hills to Home account by filling out the form
3. You’ll receive an email from Hill’s to Home with a link to activate your account and create a password
4. Jacksonville Community Pet Clinics will receive an email notification stating that you created a Hill’s to Home account and your vet will then create a nutrition plan for your pet.
5. Once your pet’s nutrition plan is created, you can log into hillstohome.com and shop any product under the nutrition plan your vet created.
